Services Provided:

Visits to homebound patients. Skilled nursing, mental health nursing, physical and occupational therapy, home health aid care, medical social work, speech language pathology therapy, nutrition counseling, trained volunteers, Medicare certified hospice care, medical supplies provided to patients. Seven day service is provided to patients. Services are provided by physician referral. CARELINE Intake/referral program provides "one call does it all: ease of intake into all services provided by Albert Gallatin. Disaster Services: As an intefral part of the human service system in southwestern Pennsylvania and northcentral West Virginia, AGHC is committed to serving our community in times of natural disaster or emergency need. In order to facilitate optimum use of our organization's resources at these critical times, AGHC participates in active preparedness to ensure the availability of our resources to those in need. In the event of a natrual disaster or emergency need sudh as' flooda, fires, destructive storms, iets., services that may be made available include registered professional nurses, licensed practical nurses, social workers, clerical counseling, home health adies, and trained volunteers. Albert Gallatin Home Care, Inc. has designated two administrative positions for coordination of liaison activity between our organization and community emergency services. The Director of Operations, Bonita Campell, holds primary responsibility for this coordination of services. In her absence, the Assistant Director of Operations, Nanette Ninos, will servie this capacity.
